Unable to Make an Appointment?

If you have an appointment with one of the doctors or nurses at Bayside Family Practice and you are unable to attend, please cancel the appointment as soon as possible so that someone else is able to take your place.

There is a link to an online form at the bottom of this page, which you can use to cancel an appointment with any doctor or nurse.

  • Respiratory Clinic - Recalls via Admin team not directly bookable.
  • Anticoagulation Clinic - Booking via Health Care Assistant (Tuesdays).

Consulting with Young People

If you are under 16 yaers of age and need to consult with a health professional in this Practice we ask you to attend with a parent, your legal guardian or a responsible adult nominated by you or your parent/guardian.

However if you wish to see the Doctor or nurse on your own this can be arranged. Sometimes the Doctor or Nurse may ask permission from you or the adult who is with you if he or she may speak to you privately.

All consultations are strictly confidential but if you are under 16 years and consult alone you will be asked if your parent/guardian is aware of the consultation.

A parent/guardian will not be told of your consultation without your informed consent unless the clinician believes that to not tell them would be harmful to you or place you in danger. We are duty bound at all times to act in your best interests and recognise your rigts and values in all consultations.
