How To Register Your Household
We can only register people living within our Practice boundary. This extends within the Furness peninsula from the outskirts of Barrow to Greenod, encompassing Ulverston town and the surrounding villages.
Please bring your medical cards to the surgery; if these have been mislaid we will still be able to help you.
As no one is automatically registered with the surgery, it is important that both you and all the members of your household register at the same time.
We recognise the important role of carers in looking after dependant relatives; please make it known if you are caring for someone.
The practice is closed to new registrations from 01/10/2008 until further notice
Who Will Be Your Doctor When you register your registration will be with the Practice. You are free to see which ever GP you prefer.
Your First Appointment When you register with the Practice you will be asked to complete a health questionnaire (one for each member of your household).
Please also make a new patient appointment with a nurse or Health Care Assistant.
This appointment is part of your registration, it introduces you to the Practice, it is an excellent opportunity to have a routine health check and will also provide the surgery with some of your medical background.
Useful information when registering includes a mobile phone number and email address, as well as a landline number and your new address if you are moving into the area.
If you move within the Practice area or if you change your telephone number or name please inform us as soon as possible.
Our Practice is now "enabled" for "GP2GP" this is an online system that allows your medical records to be automatically uploaded to our server from your old GP surgery. This will happen very rapidly if your old surgery is also enabled on this system. This will help enormously if you have ongoing medical problems and prescriptions
